Recruitment Details: Participants were recruited from the Internal Medicine Clinic at Boston Medical Center (BMC), University of Pittsburgh, University of North Carolina (UNC) at Chapel Hill in partnership with Piedmont Health and UNC HealthCare by trained and IRB-approved staff. Ways for recruiting included flyers throughout the clinics, provider referral, automated electronic medical record review, clinic sign-up sheet, and website.
