Eligibility Criteria:
Inclusion Criteria:
* Patients with complex transsphincteric anal fistulas \>18 yrs old.
Exclusion Criteria:
* Crohns fistulas
* Signs of suppuration or/and branching.
* Malignancy within 5 yrs.
* Previous radiotherapy of the abdomen and pelvis.
* Current Immune- suppressive treatment.
