Inclusion Criteria:
Montreal Cognitive Assessment score 22 or greater
* consent with observed teach back confirmation
* discharge from sub-acute rehabilitation
Exclusion Criteria:
* Montreal Cognitive Assessment score less than 22
* inability to "teach back" during consent process
* non english speaking
