* Men and women aged ≥60 years
* Discharged home with/without home care services
* French or English fluency
Exclusion Criteria:
\- Patients were excluded if they had an unstable medical, psychiatric, and post-surgical condition or a severe cognitive impairment (based on the assessment of the rehabilitation team).
