* in Northern Taiwan
* at least one of the 8 modifiable risk factors for dementia (Hearing loss, Hypertension, Obesity, Smoking, Depression, Physical inactivity, Social isolation, and Diabetes)
* living independently
* in the local community
Exclusion Criteria:
* can't speak chinese
