* Estimated Life Expectancy of 2 years

* Not primarily neuropathic pain
* Declined participation- too much traveling

* Considered unreliable- personality disorder
* Considered unreliable- cognitive impairment

* Language barrier
* Exceeded quarterly quota
* Presentation with progressive or recurrent cancer, fibromyalgia, perineal pain of unknown etiology
