* HR+ non-metastatic breast cancer
* Within 12 months of starting AI therapy
* English-speaking
* Able to provide consent
Exclusion Criteria:
* Age \<65 years
* Metastatic cancer
* Cognitive impairment preventing consent
* Receiving care at multiple participating sites
