Inclusion Criteria:
* present at least one of the following conditions: type 2 diabetes, CVD, heart failure, risk factors (smoking, obesity, hyperlipidemia, glucose intolerance, and metabolic syndrome), COPD or asthma.
* have the potential for rehabilitation
Exclusion Criteria:
* serious cognitive problems
