Eligibility Criteria:
Inclusion Criteria:
SVHs that are
* licensed and
* have electronic health records.
For SVH residents: All
* long-stay SVH residents who are
* Veterans
* who have experienced a fall in the past six months or whom SVH staff identify as at risk of falling.
Exclusion Criteria:
For SVH residents:
* Veterans under the age of 18,
* all non-Veteran residents, and
* Veterans who are comatose
* or admitted for hospice and respite care.
