* elective inguinal hernia
* lower extremity surgery
* general anesthesia
* supine position
Exclusion Criteria:
* nasal septum,
* adenotonsillar hypertrophy,
* perforated tympanic membrane,
* middle ear pathology
* receive medications for middle ear pressure .
