* Dinner or evening use of warfarin
* ≥ 3 months of continuous warfarin use
* Expectation of long-term warfarin use
* Baseline INR data made available by family physician
* Community dwelling
Exclusion Criteria:
* Patient is palliative
* Patient is unable to provide informed consent
