* BMI \> 30
* Undergoing shoulder arthroscopy in beach chair position
Exclusion Criteria:
* • Age \< 18 years
* BMI \< 30
* Known and documented \>90% occlusion of carotid artery
* Prior neck surgery
* Women who are pregnant
