* Diagnosis of SCD
* Evaluated within the 24 months prior to study entry in the hospital or clinical setting
* Expected to return episodically or regularly for care at one of the CSCCs
Exclusion Criteria:
* Deceased CSCC patient
* Inactive CSCC patient
