Inclusion Criteria:
* Hospitalized patients with primary discharge diagnosis of heart failure according to current Chinese Heart Failure Guideline were recruited from January 2012.
Exclusion Criteria:
* Hospitals were excluded if they declined to participate or did not admit patients with heart failure.No other exclusion criteria were defined.
