Inclusion Criteria:
* stage III/IV (locally advanced) carcinoma of oropharynx, oral cavity, hypopharynx, larynx, or nasopharynx
* primary treatment with chemoradiation
Exclusion Criteria:
* distant metastasis
* incomplete treatment
* adjuvant chemoradiation after surgery without apparent tumor
