* Planned hospital discharge
* Intended follow-up care, that is manageable with telemedicine
Exclusion Criteria:
* Significant mental or cognitive impairment
* Unable or not willing to sign informed consent
* Unable to understand German
* No possibility of telecommunication or use of such (app or phone)
