Population Description:
Baseline characteristics are provided for all patients who completed the cancer risk assessment screening. And for patients and primary care providers who completed a baseline survey prior to study launch. Patients, Providers, and Clinic Leaders who enrolled in the study by completing surveys post-intervention did not have Baseline data collected (2,286).
